Foundayo (Orforglipron): What It Is, How Well It Works & Who It's For

Medically reviewed by Dr. Merlin Brown, MD — Board-certified Internist | Updated October 13, 2026

Quick answer: Foundayo (orforglipron) is an FDA-approved once-daily pill for chronic weight management in adults with obesity or overweight people who also have a weight-related condition. During the 72-week period, adults experienced an average loss of 7.5% to 11.2% of body weight compared to just 2.1% who took a placebo. It works alongside diet and activity. Cost, risks and fit vary by person.

What Is Foundayo and How Does It Work?

Foundayo is the brand name for orforglipron, made by Eli Lilly. It was approved by FDA on April 1, 2026 for adults who have obesity (overweight as well as 1 or more weight-related conditions) for use with a lower calorie diet and higher physical activity.

It contains a non-peptide GLP-1 receptor agonist which activates GLP-1 receptors in the areas of the brain that control hunger and food intake. It also slows down the rate of stomach emptying, meaning that food moves out of the stomach at a slower rate.

Where other weight loss pills differ is in dosage, Foundayo is described as the first oral GLP-1 pill with no food or water timing restrictions, because it is a small molecule rather than a peptide. The Wegovy pill contains semaglutide instead, and separate trials of the two drugs cannot be directly compared.

How Well Does Foundayo Work? ATTAIN-1 Results and Cost

The main evidence behind Foundayo comes from a large clinical study called ATTAIN-1. Researchers randomly assigned 3,127 adults with obesity, but without diabetes, to take either orforglipron or a placebo (a dummy pill) for 72 weeks, along with healthy eating and physical activity. The results were published in 2025 in The New England Journal of Medicine, a leading peer-reviewed medical journal.

What the Results Showed?

All three orforglipron doses led to more weight loss than placebo. Weight loss increased as the dose increased.

Average weight change at 72 weeksAll participants (primary analysis)If everyone stayed on treatment
Orforglipron 6 mg−7.5%−7.8%
Orforglipron 12 mg−8.4%−9.3%
Orforglipron 36 mg−11.2%−12.4%
Placebo−2.1%−0.9%

Why There Are Two Sets of Numbers?

Studies often report results in more than one way. The first column is the “treatment-regimen estimand.” It includes everyone who was randomly assigned, even those who stopped taking the medicine, and it was the study’s primary analysis. The second column is the “efficacy estimand,” which estimates results if everyone had stayed on treatment. Lilly’s widely quoted figure of 12.4% comes from this second method. Both numbers are accurate. They simply answer different questions.

In practical terms, the first column is closer to what a group of real patients might experience, since not everyone continues treatment. The second shows what the medicine can do when taken as directed.

Clinical Trial Doses vs. Available Tablet Strengths

The study tested 6 mg, 12 mg and 36 mg doses. The tablets sold in pharmacies come in 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg strengths. Lilly’s medical information reports the −11.2% result under the 17.2 mg dose, so the two sets of strengths should not be compared number for number. Your doctor will choose your dose.

Averages do not predict any one person’s outcome. Some people lose more weight, and others lose less.

What Does Foundayo Cost?

Through Lilly Direct self-pay (for people paying without insurance), monthly prices are $149 for 0.8 mg, $199 for 2.5 mg, and $299 for the 5.5 mg and 9 mg doses. For 14.5 mg and 17.2 mg, the $299 price applies only if you refill within 45 days. If you do not, the price is $349.

People with eligible commercial insurance that covers Foundayo may pay as little as $25 per month. The self-pay savings card expires on December 31, 2026, so prices and eligibility may change. Confirm current terms before you start treatment.

Foundayo vs. Wegovy Pill: What Is the Difference?

Foundayo and oral Wegovy are prescription options for weight management, but they contain different active ingredients. Foundayo contains orforglipron, while Wegovy contains semaglutide.

Do not interpret the results from different clinical trials as a direct comparison. Reported weight loss may vary due to variations in patient populations, treatment length and technique. A health care provider can assist in deciding on the best one.

Who Is Foundayo Right For, and Who Should Avoid It?

Who It May Suit?

Foundayo is approved for adults with obesity, or adults who are overweight and have one or more of the following weight-related conditions: high blood pressure, high cholesterol, type 2 diabetes. It is designed for use in conjunction with a low-calorie diet and increased exercise. For individuals seeking medical assistance for long-term weight management who have not achieved their desired weight loss through lifestyle changes, it can be a viable option.

Foundayo pill is for adults only. It is not known whether it is safe and effective in children.

Who Should Avoid It?

Some people should not take Foundayo. Others should use it only after careful medical guidance.

  • A history of certain thyroid cancers: Don’t use Foundayo in people who have medullary thyroid carcinoma (MTC), a rare form of thyroid cancer, or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), an inherited rare condition. This is connected to the boxed warning on thyroid tumors, which is the most severe kind of warning on the label.
  • Pregnancy:Weight loss is not beneficial during pregnancy and Foundayo may harm the baby. Tell your doctor if you get pregnant while taking it. Label instructions include that Foundayo should be discontinued when pregnancy is detected. Tell your doctor if you are taking birth control pills to take extra precautions as Foundayo may cause them to be less effective around the beginning of your treatment and following each dose adjustment.
  • Breastfeeding:There is no information on whether Foundayo passes into breast milk, and it is not recommended for nursing mothers.
  • Other GLP-1 medicines:It is not recommended to use Foundayo pill in combination with other GLP-1 receptor agonists. Tell your doctor if you are using any weight loss or diabetes medication.

Before you start

Foundayo Side Effects, Risks, and Warning Signs

Interactions to Ask About:

  • Oral contraceptive users to change to an alternative oral contraceptive or add a barrier method for 30 days after first use and with each dose increase.
  • Foundayo may increase the level of simvastatin in the blood and may make insulin and/or sulfonylureas more likely to cause low blood sugar.

Get immediate medical attention if you have severe abdominal pain difficulty in breathing, or any signs of serious allergic reaction.

How Southdale Physicians Handles Weight Management in Edina

Weight Management is an integral part of primary care at Southdale Physicians. Since causes of obesity vary from one person to another, treatment is tailored individually.

During the first visit, a detailed look at your medical history, prior efforts to lose weight, and current medications will be done. Blood tests include an evaluation of your thyroid hormone levels, blood glucose level, cholesterol, liver enzyme profile, and hormones levels. From this information, your physician will come up with a treatment program that may entail food intake, exercise, and medications like GLP-1 drugs including semaglutide and tirzepatide. It also aims to reduce the likelihood of developing complications associated with obesity such as type 2 diabetes, hypertension, and heart disease. There is no membership charge for consultations.
